New Clinic for Seniors Opens in Stafford Legacy Community Health continues to expand services in Southwest Houston

Legacy Community Health Services, Inc. (Legacy), the largest federally qualified health center in Texas, proudly announces the opening of a second clinic designed exclusively to meet the unique health and wellness needs of seniors. The Legacy Stafford Fountains Clinic, located at 12540 Sugardale Drive in the Fountains Shopping Center, officially opens on January 10, 2022 and is Legacy’s first clinic in Fort Bend County.

The new 7,000-square-foot facility conveniently located near the 59 Southwest Freeway between the Sam Houston Parkway and U.S. Highway 90, will offer seniors primary care along with coordinated care for health and wellness needs, Medicare annual wellness and preventive services, behavioral health, chronic disease management, senior care social worker consultations, on-site labs, clinical pharmacy, same and next-day appointments, extra-long appointment times and more.

The center’s clinical team will be led by geriatrician Dr. Joanna Ira. Dr. Ira is a Baylor College of Medicine graduate who has been with Legacy for four years.

Dr. Ira focuses on the individual needs of each patient, understanding their health and wellness goals, and putting together care plans to help them reach the highest quality of life as they age. She also focuses on the management of Alzheimer’s and other dementias, falls and frailty, and the management of chronic conditions such as arthritis, diabetes and heart disease.

The remodeling of the clinic’s building was completed in seven months and includes 10 exam rooms,

3 consult areas, comfortable patient areas and a community room for classes and more. An open house for the community will be held in February 2022.

Legacy accepts Medicare, Medicare Advantage, Medicaid and most HMO/PPOs. Eligibility specialists are available to discuss sliding scale fees and potential programs to help lower the cost of health care services.
