Texas Nonprofits Unite to Launch Statewide Health and Community Data Network

Healthconnect Texas and the Patient Care Intervention Center are moving forward with a strategic unification designed to create a first-of-its-kind, statewide health and community data infrastructure for Texas.

Healthconnect Texas, a nonprofit health information network, has long focused on connecting clinical data across the state to advance interoperability, quality improvement, and better health outcomes. The Patient Care Intervention Center, also a nonprofit, brings deep expertise in community-based data that captures non-medical drivers of health, including social and environmental factors that influence wellness.
By combining their capabilities, the unified organization will link statewide clinical connectivity with community-level insights, creating a more comprehensive picture of health and wellness across Texas. This integrated approach is expected to strengthen decision-making and expand access to actionable data for healthcare providers, community-based organizations, researchers, and policymakers.
The unification is aimed at closing long-standing gaps between clinical care data and community context. With both data streams aligned, the organization will be positioned to deliver integrated solutions that support whole-person care, improve coordination between healthcare and social services, and advance more equitable health outcomes statewide.
Leaders from both organizations have emphasized that the unification reflects a shared commitment to collaboration, community engagement, and data-driven strategies that address health disparities. Additional joint initiatives and new service offerings are expected to be announced in early 2026.
